Privacy & Patient Confidentiality Agreement

This Privacy & Patient Confidentiality Agreement ("Agreement") is entered into by and between TMC Health, located at Jubail City Center, Al Jubail 35513, KSA, hereinafter referred to as "the Center," and the patient, hereinafter referred to as "the Patient."

1. Purpose and Scope

The purpose of this Agreement is to outline the policies and procedures for protecting the privacy and confidentiality of patient information during telehealth consultations provided by TMC Health.

2. Patient Consent

By utilizing the telehealth services provided by TMC Health, the Patient acknowledges and agrees to the terms outlined in this Agreement.

3. Confidentiality

The Center is committed to maintaining the confidentiality of the Patient's personal health information (PHI) in accordance with applicable laws and regulations.

4. Use and Disclosure of Patient Information

4.1 The Center will only use and disclose the Patient's PHI for the purposes of treatment, payment, and healthcare operations, and as otherwise permitted by law.

4.2 The Center will obtain explicit consent from the Patient for any other uses or disclosures of PHI not covered by this Agreement.

5. Security Measures

5.1 The Center employs industry-standard security measures to protect the confidentiality of patient information during telehealth consultations.

6. Data Retention and Disposal
The Center will retain patient records in accordance with applicable laws and regulations and will securely dispose of them when they are no longer required.
7. Patient Rights

7.1 The Patient has the right to access, review, and request amendments to their PHI.

7.2 7.2 The Patient may request restrictions on the use and disclosure of their PHI.

8. Recording of Telehealth Sessions
Telehealth sessions may be recorded for documentation, quality assurance, and training purposes. The Patient's consent for recording will be obtained prior to the session.
9. Amendment of Agreement
The Center may amend this Agreement from time to time. The Patient will be notified of any changes and provided with an updated copy.
10. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of KSA.
